어지러움에서 가장 중요한 것은 ** 뇌졸중 감별 ** 이를 위한 physical 은 HINT

HINT on Central Vertigo
1. Horizontal head impulse testing (Head Impulse)
2. Direction-changing nystagmus in eccentric gaze (Nystagmus)
3. Vertical skew (Test of Skew)

A benign HINTS exam is defined as abnormal HIT + direction-fixed horizontal nystagmus + absent skew.

첫 24시간 동안은 MRI 보다 정확할 수 있다. 

1. HINT 를 시행해야 하는 환자의 적응증

acute vestibular syndrome (AVS)를 가진 환자들

- vertigo, nystagmus, nausea/vomiting, head-motion intolerance, unsteady gait

현재 안진 혹은 증상이 있는 경우 

2. HINT 시행하기

1) Horizontal head impulse testing (Head Impulse) = HIT

없으면 central / saccade 있으면 periperal

2) Direction-changing nystagmus in eccentric gaze (Nystagmus)

Any vertical nystagmus or horizontal nystagmus / that changes direction with lateral gaze ("bidirectional nystagmus") is concerning.

바라보는 방향에 따라 안진 방향이 변화하는 경우 central 의 문제일 가능성이 있다. 

3) Vertical skew (Test of Skew)

한쪽 눈을 몇초 동안 가렸다가 재빨리 치웠을때 vertical 정렬이 제대로 되어있지 않은 경우 central 원인으로 볼 수 있다.  반복되는 검사로 쉽게 피로해지므로 많이 반복해서는 안된다.
